Individual Sessions: $125 (45-50min)

Employee Assistance Programs (EAP): Certification needed with approved rate from provider and certification number

Insurance Information
We are currently on the panels of Aetna, United HealthCare (Optum & Medicaid), AvMed (Optum), Cigna, ComPysch, Humana, Meritain Health, Oscar, and Sunshine Health (Medicaid) to name a few. Please contact our office to verify if you have mental health benefits covered by your insurance. Many times client's are responsible for a deductible, co-payment, and/or co-insurance prior to insurance covering your sessions. Our office makes the best efforts to verify all benefits prior to starting services however at times the insurance can deny services that have been rendered and client is responsible for the full session amount. You are responsible to be sure your coverage is active. If for any reason an insurance claim is denied you assume the responsibility for payment and any balance not covered by your insurance. Client must sign a "Client Responsibility Form" acknowledging that they have been informed and are in agreement with session rates and responsibilities pertaining to them if their insurance does not cover the fees.
Please note the benefits and risks before using your insurance company to cover your sessions. Your therapist will have to diagnose you with a mental disorder or issue according to the, The Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5). This diagnosis would be sent to the insurance company and recorded in your medical file. In some cases, the therapist would also have to provide additional clinical information to the insurance company, such as treatment plans, summaries, or copies of the entire record. If you are pending divorce or child custody hearings your privacy and information is less protected and could be used in court hearings if subpoenaed.
Some of our therapist have the discretion not to accept certain insurances. Our office will always inform you prior to your sessions if your therapist is accepting your insurance. If you continue to proceed services without using your insurance, you must sign acknowledgement for client payment responsibility for each session.
Disclaimer
My therapist must honor court subpoenas that require the release of specified information. My therapist may take professional action to protect those in immediate danger of physical harm. My therapist is mandated by Florida Law to report suspected child or elderly abuse or neglect. My therapist may share information with me from my children's therapy session if he or she believes that my children are in imminent danger.
I agree to notify my therapist at least 24 hours in advance should I need to cancel an appointment. If I fail to do so, I understand that I will be charged for the time I had booked, payable at my appointment at a rate of $50.00 which is not billable to my insurance with the exception of an emergency. If you are not able to make your appointment you may have the option of either a FaceTime, Telehealth or a phone session. Please consult your therapist to see which option may be available.
